Identifying Common Issues and Solutions for Clinical Genetics Centers: Discussions in the Medical Director's SIG

The ACMG Medical Directors' Special Interest Group was formed to create a forum to identify, discuss, and potentially provide solutions to the common problems faced by clinical genetics groups. We hypothesized that, despite varying administrative situations, there would be core issues that each group faced and at least attempted to solve. We felt sharing this information would be beneficial to all clinical genetics groups.

At our first meetings we identified several common challenges, and created working groups to address them. These topics addressed include:
  1. Workload. What is the expected clinical volume (number of clinics, patients, wRVUs) for a medical geneticist, genetic counselor, and advanced practice provider.

  2. Practice models. Are all MDs, GCs, and APPs based in a single administrative group? Or does each department employ their own (eg, are prenatal genetic counselors based in Clinical Genetics, or OB-Gyn?). What are the benefits and limitations of each model?

  3. Access. What are the wait times for each type of referral (cancer, prenatal, metabolic, pediatrics, adult)?   And what have you done to reduce this?

  4. Inefficiencies in practice, including uncaptured revenue. Included in this group was a discussion of how the electronic medical record has helped or hindered.

Learning Objectives

  1. Gain familiarity with the challenges that are common to all clinical genetics units
  2. Describe the different organization models among clinical genetics centers
  3. Review the barriers to care that lead to excessive wait times, and the steps taken to improve this issue
  4. Compare administrative models between clinical genetics units
  5. Identify inefficiencies that impact clinical productivity, and discuss steps taken to lessen these
